Subcision for Acne Scars: The Best Tools for Treating Deep, Depressed Scars

Deep, depressed acne scars can be frustrating, especially when the skin appears uneven or pitted. But what is one of the most effective treatments for scars caused by fibrous tissue pulling the skin downward? Is it an expensive laser treatment or a popular skin booster? One option worth considering is subcision, a minimally invasive procedure designed to release the fibrous bands beneath the skin that cause depressed scars.

The results depend not only on the procedure itself but also on the type of scar, the depth of the underlying adhesions, and the technique and instruments used. Understanding these differences is essential when choosing an appropriate acne scar treatment.

Subcision for Acne Scars: The Best Tools for Treating Deep, Depressed Scars

What Is Subcision for Acne Scars?

Subcision is a minimally invasive procedure that uses a needle or specialized instrument inserted through a small entry point to release fibrous bands beneath the skin.

After severe acne, some people develop fibrous scar tissue that attaches the skin to deeper tissues. These bands pull the skin downward, creating depressions commonly associated with rolling acne scars and other types of atrophic acne scars.

Subcision works by cutting or releasing these fibrous connections, allowing the depressed skin to move more freely. The release may produce an immediate improvement in the appearance of some scars.

The controlled tissue injury can also stimulate the body’s wound-healing response and collagen remodeling over time. However, the degree of improvement varies depending on scar type, depth, and individual healing characteristics.

1. Nokor Needle: Releasing Dense Fibrous Adhesions

The Nokor needle has a triangular cutting tip designed to cut through dense fibrous bands beneath the skin.

It can be useful for selected scars with firm, established adhesions that require direct mechanical release. However, because of its sharp cutting edge, there is a risk of bruising, bleeding, and hematoma formation.

Careful technique and appropriate patient selection are essential to minimize complications.

2. Standard Injection Needles: Versatile and Precise

Conventional injection needles are also used for subcision in selected cases.

Their size and flexibility allow clinicians to approach fibrous adhesions at different depths and angles. They can be useful when treating localized areas that require controlled release.

However, dense or extensive adhesions may be difficult to release completely with a standard needle alone. Depending on the scar’s characteristics, additional treatment sessions or a different instrument may be needed.

3. Blunt Cannulas: Treating Wider Areas

Blunt cannulas have rounded tips rather than sharp cutting edges. They can be used to separate tissue planes and release certain fibrous adhesions across a wider area through a small entry point.

Compared with sharp instruments, blunt cannulas may reduce the risk of injury to some blood vessels and surrounding structures. They may also be associated with less bruising in selected cases, although bleeding and other complications can still occur.

Their main limitation is that particularly dense fibrous bands may be difficult to release with a blunt instrument alone.

4. Taylor Liberator: Targeting Strong Fibrous Bands

The Taylor Liberator is a specialized instrument designed to engage and release resistant fibrous scar bands.

Unlike instruments that primarily separate tissue planes, its notched design can help capture and mechanically disrupt certain adhesions.

It may be considered for selected scars with dense or extensive fibrous attachments. However, its use requires considerable anatomical knowledge and technical experience.

Because deeper structures may be injured if the instrument is used incorrectly, careful control of the treatment depth and direction is essential.

5. Trifill: Using Pressurized CO₂ for Tissue Release

Trifill is a device-based approach that uses pressurized carbon dioxide to expand tissue and help separate certain adhesions. The resulting tissue response may also contribute to collagen remodeling.

It may be considered for selected depressed scars, including some broader rolling scars. Its suitability depends on the location, depth, and severity of the adhesions.

For very dense fibrous bands, this approach may not provide sufficient mechanical release on its own, and other techniques may need to be considered.

6. Needle-Free Injection Devices: Supporting Scar Treatment

Needle-free injection devices, such as systems marketed under names including PuriJet, Mirajet, and SkinJet, use high-pressure fluid jets to deliver substances into the skin.

Depending on the device and treatment protocol, this technology may help deliver selected solutions into targeted tissue layers and may be used as part of a broader skin-rejuvenation or scar-treatment approach.

Some protocols involve substances intended to support hydration or skin quality, such as hyaluronic acid-based formulations. Other injectable products may have different compositions and indications.

However, needle-free delivery should not be assumed to mechanically release deep, dense fibrous bands as effectively as direct subcision. Its role depends on the device, the substance being delivered, and the specific scar being treated.

Why Scar Depth Matters in Subcision

Not all depressed acne scars are the same. Fibrous adhesions can vary in strength, width, and depth, extending through different layers of the skin and into the underlying tissue.

A technique that works well for a broad, shallow scar may be less effective for a deep scar with dense adhesions.

For this reason, some clinicians may consider a combination of instruments rather than relying on a single approach. The goal is to match the treatment method to the specific characteristics of each scar while minimizing unnecessary tissue injury.

What Is Multi-Layer Subcision?

Multi-layer subcision involves addressing adhesions at different tissue depths according to the patient’s scar pattern.

For example, a blunt cannula may be used to separate broader tissue planes where appropriate. If certain dense bands remain, a sharper instrument may be considered for more targeted release. A conventional needle may then be used to address selected residual adhesions.

This approach aims to treat both broader areas of tethering and more localized fibrous connections.

However, combining multiple instruments does not automatically guarantee better results. The appropriate method depends on the scar type, anatomical location, treatment risks, and the clinician’s assessment.

Is Subcision Better Than Laser Treatments or Skin Boosters?

Subcision, laser resurfacing, and injectable skin treatments address different aspects of acne scarring.

  • Subcision: Targets fibrous bands that pull the skin downward.
  • Fractional laser treatments: Aim to improve skin texture and stimulate collagen remodeling.
  • Skin boosters: May support hydration or improve selected aspects of skin quality, depending on the product.

For scars caused primarily by strong fibrous tethering, releasing those attachments may be an important part of treatment. Other scars may respond better to laser resurfacing or a combination of approaches.

No single procedure is the best option for every patient. Treatment should be selected according to scar morphology, skin type, medical history, and the risk of complications such as bruising, infection, pigmentation changes, or further scarring.

Choosing the Right Subcision Technique for Your Acne Scars

Successful acne scar treatment requires more than choosing an expensive device or a particular needle. It requires an accurate assessment of the scar’s depth, the extent of fibrous tethering, and the surrounding anatomy.

Different instruments have different strengths and limitations. In selected cases, combining techniques may allow a clinician to address adhesions at multiple depths, but the approach must be individualized.

If you have deep, depressed acne scars, consult a qualified medical professional experienced in acne scar treatment. A personalized assessment can help determine whether subcision, laser treatment, injectable treatments, or a combination of approaches is appropriate for your skin.
